Jane Teasdale will discuss the importance of social connection for individuals and family caregivers in our communities. In her presentation she will also discuss social prescribing, person centered care model through Mosaic (The Meaning of Me®) and community mapping to stay connected.

Jane Teasdale is well known for developing awareness of home and health care issues in the community and for developing relations between healthcare professionals, home care providers, not-for-profit agencies and other important services providers that are needed to provide the wider levels of support often required by those in need in the community. Mosaic Home Care & Community Resource Centres offers an award-winning model of care (Gold Award, European Society for Person Centered Healthcare) that is unique to the homecare industry. It operates community resources centres across the Greater Toronto Area: these centres not only provide information on both for profit and not for profit services and supports to families and individuals in need of assistance but regular educational and social events. Jane Teasdale is also co chair of the North York Elder Abuse Network. She was also presented at the International Federation on Ageing and at NICE Conference on Mosaic’s integrated model.
