Mosaic provides medical and non-medical services in the Greater Toronto Area, including York Region, to enhance the lives of those who might otherwise have to move to a retirement home or long term care. Whether a child, young adult, parent or grandparent, we provide solutions that allow you or your loved one the ability to be independent and live in your own home and community.
In addition, we provide relief for family caregivers, new and expectant mothers, veterans, and all those needing short term care after surgery and who might not be able to look after themselves. We are also here to help reintegrate people into the community, socialize, and enjoy their lives.
At Mosaic, we also understand that you or your loved one may be in need of more than homecare. We believe that lifestyle is an important aspect to the health and well being of our clients. It is why we have created the Mosaic Club, where you can enjoy activities and interests in the company of others in a warm and welcoming environment.
Our team of professional caregivers is available to provide live in care, assistance by the hour or day and to accompany you to activities and interests in the community and at our Mosaic Club. It is how we make a difference in your life.
Personal care and support services
Mosaic provides medical and non-medical personal care and support services in any environment, whether at home, on holiday, in a hospital, or in retirement homes and long term care facilities. We also provide weekend support for sponsorship cases already in place.
We provide services for people of all ages and needs including palliative care patients, those with Alzheimer’s and dementia, Parkinson’s and support for people with disabilities.
Our non-medical personal care and support services include, but are not limited to the following:
- Monitoring of medication and medication pick-up
- Helping with bathing, and transferring & positioning in bed and other personal needs
- Assisting with discharge from hospital and trips to the doctors.
- Providing companionship
- We provide care by the hour, overnight, 24 hour and live in care
We also ensure that families and individuals have access to all the information they need to help them make the care decisions they need. This support is provided by our resource centre and our care co-ordinators.
These services help assist individuals with the tasks of daily living relating to household management. The goal is to provide relief for family caregivers and to help preserve and maintain a safe, healthy and sanitary home for the client.
Services include but are not limited to the following:
- Grocery shopping and other errands
- Light house cleaning: vacuuming, dusting, cleaning the kitchen and washrooms, laundry
- Meal planning and preparation
- Monitoring of client’s physical and mental well being
At Mosaic we realise that lifestyle, social interaction and positive mental health in combination with personal care and support are key to enhancing the lives of those we serve.
Lifestyle options and planning are discussed at our assessments, but will include engaging clients, according to their personal situation, in social interaction, excursions to parks, movies, art galleries, community groups, theatre, family & social events and organised events in and around the community for caregivers and clients. Additional charges may apply, but all clients are automatically enrolled, without charge, as members of our Mosaic club, a free lifestyle option for our clients.
